Osakidetza adopts a new standard for congenital coagulopathies

The Basque public health system, Osakidetza, has just announced the adoption of a new standard of care for patients with congenital coagulopathies. This decision, made public on March 16, 2026, is part of a desire to optimize the quality of treatments and personalize therapeutic support. Health authorities emphasize that this initiative responds to a growing need for harmonization of practices within hospitals in the region.

The CMO model, now integrated into the Osakidetza protocols, is based on a collaborative approach between doctors, pharmacists and patients. It aims to standardize monitoring procedures while allowing individualized adjustments according to the evolution of each case. This methodology, developed by the Spanish Society of Hospital Pharmacy, has already proven itself in other autonomous communities, demonstrating a reduction in complications linked to anticoagulant treatments.

Congenital coagulopathies, such as hemophilia or Willebrand disease, affect several thousand people in Spain, including a significant proportion in the Basque Country. These pathologies require rigorous treatment management, often for life, to prevent hemorrhagic or thrombotic risks. The introduction of this new standard will ensure optimal continuity of care, from diagnosis to long-term follow-up.

Osakidetza specified that the training of the health professionals concerned will begin in the second quarter of 2026. Refresher sessions will be organized in the main hospital centers, notably in Bilbao, San Sebastián and Vitoria-Gasteiz. This step is considered crucial to ensure uniform implementation of the model throughout the Basque territory, taking into account local specificities.

The implications of this reform go beyond the strictly medical framework. Indeed, better management of coagulopathies could lead to a reduction in unscheduled hospitalizations, thus relieving pressure on emergency services. In addition, standardization of procedures will facilitate the collection of epidemiological data, essential to guide future public health policies regarding rare diseases.

Patient associations welcomed this announcement, while calling for increased vigilance during the transition phase. They point out that equitable access to innovative treatments remains a major issue, particularly in rural areas where the supply of specialized care may be limited. Osakidetza is committed to regularly evaluating the impact of this new standard, with the first assessments planned for the end of 2027.

This initiative aligns with European recommendations for the management of chronic diseases, aiming to improve the quality of life of patients while optimizing health resources. It intervenes in a context where health systems face growing challenges, linked to the aging of the population and the emergence of new costly therapies.

The adoption of the CMO model by Osakidetza therefore represents a significant step in the evolution of pharmaceutical care in Spain. It illustrates the capacity of regional health services to innovate while strengthening coordination between the different actors in the care pathway. The coming months will allow us to concretely measure the benefits of this approach for Basque patients suffering from congenital coagulopathies.
